Pat Jakubiec, President and Owner of Vita Cura LLC, presented her evidenced informed integrated care program and an overview of the Allen cognitive disability model. Pat emphasized the critical importance of the context of care when using the program.  Vita Cura provides onsite organizational development that is tailored to each individual organization.

  Three organizations implementing the program shared their results.  Beth Toler, resident service coordinator from Englewood Colorado talked about how falls, weight loss, use of supplements and behavior challenges have been dramatically reduced in her long-term care setting. Judy Johnson from Boise Idaho is an advanced practice psychiatric nurse talked about using this approach in homecare. She described how she works with the care team and how they have expanded services to underserved populations. Kurt Heusser, Physical Therapist, & Rehabilitation Director, Salt Lake City Utah described how the staff in his long-term care setting are empowered. He reported that falls, behavior challenges, restraints and the use of psychotropic medications have been reduced since implementing the program. Both Long-term care facilities have reduced falls by 2/3rds on average.
